Evaluation Forms

Administrate provides a standard PDF evaluation form for your courses with a set of predefined questions. This PDF can be attached to student communications or printed out from the system. Please note that the PDF content is static, i.e., the questions cannot be answered in the PDF. It requires printing to complete. Some customers choose to customize the questions.

Requirements

Please note that the design of the PDF is not customizable, only the wording of the questions and the groupings’ headers. We support feedback options of a ratings scale 1-x (up to x=10), standalone check boxes, and open text fields.

If you would like to customize the evaluation form, please provide us the desired updates to the questions and/or the question headings.
